- Recent passport size photograph. Matriculation certificate issued by Board/University for proof of date of birth.
2.Class-Xll certificate with mark sheet. di Degree certificate with mark sheet issued by the Assistant Registrar/Registrar/Controller of Examination/Vice-Chancellor of the concerned University to ascertain educational qualification.
3. Diploma certificate with mark sheet of Library Science from recognized University.
4. APST certificate for APST candidates.
5. intimation letter to HoD/NoC for those who are in regular Govt. service.
