Information about How to obtain a disability certificate
A differently-abled person can get disability certificate from the Directorate Health Services, Assam. There are different application forms for different categories of disabilities like intellectual or learning disability, psychiatric disability, visual impairment, hearing impairment, neurological disability, locomotor/orthopaedic disability etc that can be obtained both in specified hospitals in districts and the Social Welfare Department. A filled in application form has to be taken to the specified medical board (at MMCH in Kamrup-Metro) where specialist doctors concerned ascertain the percentage of disability on the application form itself. The applicant – who may be the candidate himself/herself or his/her legal guardian – will have to submit this form to the Joint Director of Health Services of the respective districts for his/her counter signature. The counter-singed application form itself becomes the disability certificates. Only people with forty or above percentage of disability are entitled to get government benefits meant for physically challenged people.
The applicant has to provide his/her age proof, residential proof and two copies of passport size photos.
