Registration of PwDs for UDID cards

The Directorate of Social Justice and Empowerment, Assam, appealed to all persons with disabilities (PwDs) to apply online through http://www.swavlambancard.gov.in to the Department of Empowerment of Persons with Disabilities under the Union Ministry of Social Justice and Empowerment to register themselves for UDID (Unique Disabilities Identity) cards.

PwDs who have obtained certificates from the State Health Department will have to upload a copy of a passport-size photo of the applicants, the address proofs, identity proofs, disability certificates, personal details, details of disabilities and identity details.

Applicants who do not have certificates from the State Health Department will have to upload their passport-size photos, address proofs and identity proofs. Willing persons can contact the Directorate of Social Justice and Empowerment, Beltola, Guwahati.
